Clinical Services

The 24-hour nursing care provided at Advocate Sherman West Court follows the highest quality of standards. Working closely under the direction of, and in partnership with attending physicians, our nursing staff provides care and services that respond to the total well-being of each patient. Advocate Sherman West Court has an experienced nursing staff with a low nurse-to-patient ratio.


Daily nursing services include but are not limited to:

Nutrition Services

Advocate Sherman West Court enhances the quality of life for our patients by providing therapeutic diets, proper nutrition and well-balanced meals. Physician prescribed, therapeutic diets are developed by our registered dietitian based on individual patient needs and the latest medical nutrition therapy.

 

Case Management

At Advocate Sherman West Court, we understand the challenges of returning to the community after weeks or months of rehabilitation. Our professional social worker and nurse discharge planners are here to answer any questions or concerns with compassion and individualized attention. They also offer counseling, support and assistance with discharge planning to help both you and your loved ones with the transition from Advocate Sherman West Court to home and work life. This is just one element of our commitment to patient-centered service throughout the full continuum of care.

 

Accreditation and Licensing

Some important things to think about when choosing a skilled nursing or rehabilitation facility is its Accreditation and Licensing.

Is the facility accredited by “The Joint Commission”?  Being accredited by The Joint Commission is an optional program for skilled nursing facilities and is like having a “Gold Seal of Approval™”.  The Joint Commission’s “Gold Seal of Approval™” is an internationally recognized symbol of quality. Organizations that have proven its commitment to high quality patient care and its willingness to be measured against the highest and most rigorous standards of performance. Forward-thinking health care organizations volunteer for evaluations by The Joint Commission to stimulate improvement efforts in health care quality and patient safety and to help support operational efficiencies.

Advocate Sherman West Court is accredited by the Joint Commission and meets the criteria and standards set for higher quality of care. Information regarding facility accreditation can be found at www.jointcommission.org on the web. Select “Quality Check”.

Advocate Sherman West Court is also licensed by the State of Illinois and governed by local and federal guidelines. If you are receiving skilled care, and Medicare or Medicaid is paying for your care, verify that the facility is Medicare and Medicaid Certified. Advocate Sherman West Court is certified and licensed with Medicare, Medicaid and Private (self pay) beds. Our facility license specifies a limited number of beds for each.


“Nursing Home Compare” at www.medicare.gov will provide you with information regarding information such as:

  2. Staffing information
  3. Information about the residents
  4. Facility inspection results
  5. Quality information

While it is important and convenient to verify accreditation and licensing on-line when comparing facilities, we highly recommend a facility tour when making your decision.

  

Attending Physicians, Consulting Physicians, and Medical Directorship

Advocate Sherman West Court is accredited by the Joint Commission which requires that all physicians providing care or consultative services be credentialed and privileged through our Health System.  Sherman currently has over 700+ physicians on staff, many willing to follow your care in our facility.

Advocate Sherman West Court also has a Medical Director who works closely with administration and is the driving force for performance improvement and the leading medical voice in outcomes management. This is accomplished by adapting methods of concurrent monitoring and evaluation of outcomes data and best practice ensuring that the highest quality standards are continuously met.
